Forms

Forms must be submitted by the FIRST DAY OF SCHOOL OR CAMP!

If you have already submitted these forms, we DO NOT NEED them unless there are updates.

• • Bring in photo ID of your child: birth certificate or passport

  • School Year – Parent Permissions Form and Student Questionnaire

  • Camp – Parent Agreement 

  • School Entrance Health Form (signed by a doctor)

  • Authorization for Emergency Medical Treatment

  • Medication Authorization Form (Only if applicable)
